Are you unable to leave your house due to a
physical disease or disorder? Do you have a mental disease or disorder that
causes you to be afraid to leave home? Have you been diagnosed with
agoraphobia, obsessive-compulsive disorder, or another rare disease that causes
you to live in hiding?
Has
your disorder or disease affected your relationships and ability to communicate
with the outside world? Do you fear crowds or strangers and refuse to leave
your home due to this? Do you wish to overcome your personal, physical or
mental struggle?
If you appear to be between the ages of 18 and 26 and
