Shauna is an athlete and although she knew she "had bigger features" she didn't consider her nose one of them until after her pregnancies. That's when she said her nose great bigger and bigger. She decided that a cheap trip to Mexico would solve all of her problems. Because that sounds like a great plan. When she woke up the tip of her nose was purple and a sharp piece was still attached. She went back four months later for a subsequent surgery. As soon as she took off the dressing she noticed "dripping". Basically she snots all over the place constantly. Goodbye make-out sessions. Dr. Nassif wants to do an open rhinoplasty, open up the valves, and reconstruct the tip. To be fair, she looked beautiful before her surgeries, and she looks beautiful now. But if snot bothers her, why not wipe it once and for all? It turns out he couldn't find anything in Shauna's anatomy that would be the cause of the dripping, so it could be attributed to allergies. But only time will tell. As it would turn out, the tissues are no longer needed.

Ilana had a boyfriend obsessed with large implants. In order to please him, Ilana decided to go to a 38F and is now up to a 38GG. It turns out even after two surgeries she still wasn't quite what he wanted. Isn't that always the way? Her back and neck hurt constantly and now is experiencing cramping in her arm pit so she's ready for a reduction. Dr. Dubrow, upon first inspection, can already tell she has underlying tissue trauma. While he's game to reduce the areola and the implant size, he's also willing to do something the has only done once before... pulling fluid out of the implants with a needle in order to soften the tissue. Then after everything has relaxed he can go in and do the reduction surgery. That way the risks of anesthesia and surgery are minimized. Part 1 involved removing the fluid in the implant. Once deflated, Dr. Dubrow realizes just how thin her skin is and is concerned about the safety of future implants. But I bet she can breathe better. Part 2 is assessing tissue and scar tissue, then placing in sizer implants and decide whether or not a lift is a good idea. Before surgery Ilana said she'd like to be a DD, but during surgery Dr. Dubrow decides that going that large isn't working for her body. He makes an executive decision to put in the larger implants so that he doesn't have to do a lift, that way he gets a safe result and a happy patient. In a new turn, Ilana doesn't have a reveal to friends and family, she instead meets Dr. Dubrow in his office to show off her new, braless, breasts. And she should because now the weight has been lifted off of her chest, literally.

Roy is on the quest for a perfect Michael Jackson nose. He grew up strictly religious with no television or music. Then he found Michael Jackson's music. It gave him hope in life and after MJ's passing he wanted to pay tribute to his idol by becoming an impersonator. In order to pull this off he wants MJ's "perfect, sculptured, made nose". Yeah... I don't know too many people who are looking at Michael's skinny nose as a work of art but, hey. The best part of the consultation is Terry calling Paul Bubbles the Chimp. Ha! When the doctors explain that Michael Jackson's nose was basically y gone and he couldn't breathe, Roy isn't discouraged. In fact, that's exactly what he's going for. Except he calls it "being authentic". But eventually Dr. Nassif is able to convince Roy that a dead nose is not the best nose and the surgery is too risky. No surgery for you, back to contouring.
