Born a male, Carmen knew at 10 years old she wanted to be female, even going to therapy in case it was a phase. After high school she moved to Atlanta and started a more formal transition. Except she didn't stay in the United States for her surgery, she instead went to Guadalajara for a new nose (that she picked off a menu), tighter face, and big boobs. She was happy with everything except for the nose. The next surgery was to fix the nostrils but no surgeries ever fixed the problems. Carmen wants to remember what it's like to breathe through her nose so she's asking for Dr. Nassif's help. During the pre-op consultation, Dr. Nassif finds that she barely has enough cartilage from her ears to rebuild her nose and he is concerned that her nostrils are too collapsed. Dr. Nassif's plan is to remove fascia from her scalp, cartilage form her ear and ribcage and then re-sculpt the nose. During surgery he finds that the nostrils are severely collapsed and there is a lot of scar tissue, which is making the surgery difficult. Apparently during Carmen's four surgeries, they never fixed her deviated septum. In fact, one of the surgeons left a scar band that was blocking her airway. In the end, everything worked out and Carmen looks amazing and can actually breathe again.

Maria chose a double mastectomy because she tested positive for the BRCA1 gene mutation. Her possibility of getting cancer was 87% so she chose to remove her breasts to save her life. After the surgery, her plastic surgeon discussed nipple banking (transplanting them to her lower abdomen until she got her implants put in. When she went back in to check on her expanders in her breasts, she found that the tissue had died and she needed a revision surgery. 6 months of recovery later, her implants were put in and nipples replaced and she hates them. Maria calls her breasts "frankenboobs" and wants them fixed so that she can feel like a beautiful woman again. During her pre-op consultation Dr. Dubrow finds that she has every complication one can have after a breast augmentation. His plan is to take the implants out, reinforce the pocket, put in new implants and then do 3D tattooing for new nipples. During surgery Dr. Dubrow finds that she has had a rupture in one of her breasts and cadaver skin that has rolled up instead of laying flat. Gives new meaning to her "frankenboobs". Next stop, 3D nipples. Her results are so great that she's is emotional and now considers herself complete and whole. Now that she's satisfied and confident she has promised to have her daughters tested for the cancer gene.

Star loves being fake. In fact, she hates anything and everything natural. Self admittedly she was a pretty girl but she wanted bigger breasts. Then her mother (a Jehovahs Witness) called her a prostitute. So she left home and started getting more surgeries so she would no longer look like her mother. She has had: botox in hair and chin, lip for legs, lip fillers, designer vagina, 2 breast augmentations and she wants them bigger, and a rhinoplasty. While she ants to breathe better, she also wants a more manufactured look. Dr. Dubrow fears that Star may have symmastia and Dr. Nassif thinks her nose has collapsed in the middle. During her pre-op appointment they feels that she is not suffering from symmastia and she refuses to have a bigger nose so she is refusing the rhinoplasty. Since her functional goals and her cosmetic goals don't match, the doctors will not be performing surgeries for her. She's off to Poland to find a new doctor.
