All ethnicities, genders. Ages 19-45 Y/O.
Must have used HealthCare.gov in 2021 [2/15/21-8/15/21] to sign up for a new plan or re-enroll/re-evaluate old plan
MUST SUBMIT via online form: https://form.jotform.com/212223851332142
We are especially looking to include families with 1-2 children, all must be on a health plan via HealthCare.gov
